LSA Fall Sports Update
All athletes must have a copy of a Physical Examination by a doctor and Medical History Form on file in the Athletic Office prior to participating in a practice. You must have also completed the online Medical Consent. The Physical Examination and Medical History Form can be found in the athletic section of the LSA website or click here for the link.
Please see the directions for completing the online Medical Consent Form at the bottom of this email.
High School Football (Grades 9-12)
Practices will begin on Thursday, August 1. Practice times are as follows:
August 1 – 8am to 10am player meeting and equipment checkout; 5:30pm to 8:30pm practice
August 2 – 5:30 to 8:30 practice
August 3 – Team Service Project
August 4 – Off – Sunday
August 5 – 5:30 to 8:30
August 6 – 5:30 to 8:30
August 7 – 5:30 to 8:30
August 8 – 5:30 to 8:30
August 9 – 5:30 to 8:30

Middle School Boys Football (Grades 6-8)
Practices will begin on Wednesday, August 1, from 6:00-8:00pm. Coaches will be available 4:00-6:00 pm to hand out equipment. Athletes should come dressed in shorts and t-shirt and should bring a pair of cleats to the first practice. ,
Practices will be held 6:00-8:00 pm on the weekdays until school starts on August 15.

There will be open gym times August 1 and 2, 6:00- 7:30pm.
Tryouts will be Monday, August 5, and Wednesday, August 7, 6:00-7:30pm. Athletes should wear knee-pads.

The best way to demonstrate your school spirit and support your child is to join or renew your membership in LSA's Booster Club. As a member, you can enjoy these benefits:
- Family admission to ALL home LSA Athletic events. Why pay $8 for admission at each game for each person in your family when you can attend all home games for all sports just by showing your Booster Club pass?(all membership levels. Note the South White level is only $150!)
- "Boosters Reserved Seating" at all home football games(And designated seating is also a benefit of Pioneer and Pioneer Plus)
- "Booster Bucks" to buy anything offered in the LSA Spirit Store(amount depends on level)
- Designated Parking Space Pass for home football games(Pioneer Plus level)
- Recognition in the All Sports Program* distributed in the Fall (Pioneer Plus, Pioneer and South Green). Free Program for Pioneer and Pioneer Plus members.

All Sports Program
The Athletic Booster Club will be putting together an All Sports Program that will be distributed all year at home athletic contests during the fall, winter and spring. This publication will be highlighting all of our high school and middle school sports teams and athletes including: Football, Volleyball, Cross Country, Basketball, Soccer, Swim, Baseball, Softball, Tennis, Golf, Track & Field, Cheer, Dance in one program. We will also do ads for our club sports teams: Boys Volleyball, Sailing and Fishing.
Advertisement space will be sold throughout the program to cover the cost of the program and to raise money for LSA sports teams. This year we will have a variety of ad space available for businesses, family, and personalized messages of encouragement supporting individual teams and athletes. Instructions for Medical Consent Waiver Forms
ACTION NEEDED: 19-20 Medical Consent Information (REQUIRED)
LSA uses the Electronic Medical Consent feature located within the Renweb School Management System. This process expedites the retrieval of information for your student in the event of a medical emergency. Each student will need to have their information updated and completed in Renweb by August 2, 2019. Please use the following instructions to complete your student’s medical information.
Instructions for Medical Consent:
·Log-In to Parents Web
·Click Web Forms
·Click Edit Family Information Form
·Click Enrolled Student Information
·Completely fill out the MEDICAL section of this page.
oIf you wish to give the school consent to treat in the event of an emergency, you must choose that option (highly recommended).
oEvery box needs to be completed including all insurance information.
o If your student does not have a medical condition or allergies, please put “NONE” in the corresponding box.
·Click “SAVE” at the bottom of the page when finished
